If you have enabled auditing for account management in the default domain
controllers policy or another GPO on that OU, you should find event id's
627 and 628:

To see how often the password was changed i think not. You can try to use
ldp.exe to search for the user properties stored in the AD database. See

Do you have account management enabled in your GPO? Otherwise you can not
see it. Check for event id 627 or 628.
